We are replacing some old warped and rotted trim boards. Should we paint and prime all sides before putting them up to help seal them in the event water gets behind them? Or is it sufficient to just prime the backs and then caulk them once attached?

This is an excellent idea. It is called "back priming". The practice is priming all sides and the cut ends before installation. After installation the trim is caulked and then painted.
It isn't necessary to paint the back. The primer will seal it and protect your investment.
